STAFF NURSE (Hougang)

The jobholder is responsible to provide personalised and cost-effective care to the patients in the nursing home in accordance to established policies and procedures, standards of nursing care and practices. He/she deputizes the NM in her absence.

Job Description

    • Complies with approved standards of nursing practices.
    • Provides nursing services requiring substantial and specialised nursing skills.
    • Works closely with patient’s doctor and multidisciplinary team to provide care to the patients.
    • Updates on changes in patient’s conditions and needs.
    • Ensures care delivery and nursing interventions as outlined in the care plan.
    • Evaluates the patient’s response to care interventions and recommends revision of care plan as necessary.
    • Works with Allied Health professionals and/or initiate preventive and rehabilitative nursing procedures.
    • Carries out and supervise assessments of new admissions.
    • Prepares and administers medications to residents and comply with safe medication practices.
    • Responsible for safe keeping of control drugs.
    • Participates in training and development of care staff.
    • Manages any emergency that arises and ensure prompt actions to mitigate the situation and escalate promptly to supervisor, where necessary.
    • Works with staff to maintain a safe and clean environment in the nursing home.
    • Participates in quality assurance activities to monitor and evaluate the quality and appropriateness of nursing care.
    • Participates in quality audits that are planned and conducted periodically.
    • Contributes to the evaluation of staff performance appraisal with Nurse Manager.
      Deputizes the Nursing Manager in his/her absence.

    REQUIREMENTS

  • Min Diploma in Nursing.
  • Possess valid SNB license.
  • Good communication skills with a strong passion to work with the elderly.
  • Able to work 3 rotating shifts.
  • NURSING AIDE
    The jobholder supports and assists the RN/ENs in the delivery of care to patients in the nursing home in accordance to established policies and procedures, standards of nursing care and practices.

    Job Description

    • Complies with approved standards of nursing practices.
    • Participates in activities to improve quality of care.
    • Attends to the needs of the patients.
    • Reports patient’s needs to the nurse in charge.
    • Ensures the safety, comfort and well-being of the patients.
    • Ensures good personal hygiene of the patients under his/her care.
    • Checks, monitors and records vital signs taken and to report to nurse in charge any abnormal readings.
    • Assists in the transfer and positioning of non-ambulant patients.
    • Assists the RN in ensuring patients take their medications.
    • Serves meals and feeds the patients.
    • Assists the nurse in the care for patient with urinary catheter.
    • Assists in admission and orientation of new patients.
    • Maintains accurate documentation of care given.
    • Accompanies patients to the hospital/polyclinic for follow-up appointments and outing trips.
    • Maintains a safe and clean environment for the nursing home.
    • Performs rehabilitation activities such as motion exercise and walking with patients.
    • Organizes and sends patient for appropriate therapy as prescribed, example occupational therapy.
    • Escalates to nurses any emergency situations; where necessary take first responder actions to mitigate the emergency.
    • Supports in clinical service projects.
    • Supports infection control campaign activities.
    • Assumes additional responsibilities as assigned.
  • SENIOR CARE ASSOCIATE
    Responsible to provide nursing care at home for recently discharged patients in their homes, while they await arrival of foreign domestic workers or other long term caregiving arrangements. To provide personal services for clients requiring care to continue living at home and in the community and support caregivers in the care of their frail and home-bound seniors. This include services such as assistance in performing Activities of Daily Living, assistance with medication, mind-stimulating activities, elder-sitting and other personal care tasks that best meet the clients’ needs.

    Job Description

    • Provide companionship and Recreational Activities for elderly.
    • Perform Personal Hygiene (Showering / Bed Bath / Dressing / Diaper change).
    • Observing, monitoring and recording patients’ conditions by taking temperatures, pulse, respirations, intake and output etc.
    • Communication with patients, relatives and carers
    • Personal care including infection prevention and control, food, personal hygiene and overall reassurance, comfort and safety
    • Promoting positive mental/physical/nutritional health with patients
    • Support the client with his specific household tasks, including preparing meals, doing laundry, changing bed sheets, making sure the client is in a clean environment (cleaning their room and bathroom etc.)
    • Assist the client and carers in checking and ordering supplies
    • Any other ad hoc projects as assigned by supervisor
  • THERAPY AIDE
    Supports and assists the Allied Health Department in the therapy sessions for the clients of the Senior Care Centre.

    Job Description

    • To assist the Therapists in implementing rehabilitation programme and therapy works to residents/clients in the nursing home / Centre.
    • Helps treat residents/clients by applying heat packs; assisting residents onto exercise equipment, monitoring motion; tracking walking time and distance; measuring mobility; performing prescribed exercises and strengthening techniques.
    • Provides equipment and supplies by sterilizing and delivering equipment and supplies to treatment area; positioning equipment for therapist access; positioning residents / clients on equipment.
  • REQUIREMENTS

    • Certificate in Therapy Services preferably HMI or WDA Certification.
    • Creating a Safe and Effective Environment, Verbal Communication in dialects is an advantage.
    • Have Listening and Coaching skills.
